Rules

Sweepstakes is free and open to anyone anywhere in the world who will be at least 18 years old and no more than 89 years old on July 1, 2007 (or current contest year). Void where prohibited.

To play: Fill out the registration form completely including date of birth (month, day and year) and click on the Register button. Entries are limited to one per person using this registration form. Any multiple or subsequent entries by the same contestant into the TheBirthdayGame.comTM which are obtained in any other fashion are invalid and will be discarded. You should receive an Email from us within 24 hours confirming your sign-up. (If you don't, please register again. Check your Email address carefully and remember to click on the Register button once you've filled in the form.) Forms containing incomplete or inaccurate information will be disqualified. Neither ID Promotions, EZsweeps.com, or TheBirthdayGame.com will be responsible for any entries not properly or accurately transmitted or received due to any cause whatsoever.

Prizes for the daily, monthly and annual drawing will be selected by ID Promotions. ID Promotions, Inc. is the insurer of daily and monthly prizes. Annual grand prizes are insured by the current sponsors. For the annual prize, a random drawing to be held prior to July 1, 2008 (or July 1st of the current annual sweepstakes) and one entry will be selected from all valid entries received prior to the sweepstakes deadline. The selected entry will be assigned an envelope and will have the opportunity to match the birthday contained in that envelope. For those members with the birthday (month, day and year) on the selected entry that matches the predetermined winning birthday, a drawing will be held for the grand prize. In any of these events, the entrant will be declared the winner pending presentation and verification of proper identification, including a valid driver's license and birth certificate. The date of birth and contestant's name and address on the identification must match exactly with the date of birth provided on the sweepstakes entry. There may be a maximum of one winner. Residents of Canada must successfully complete a skills test before being awarded any prize. Void in Quebec and wherever else prohibited by law.

In addition to the daily, monthly and annual grand prizes, there are referral prizes as well. These prizes will be awarded to any person who, or website which, refers, as defined below, a new person (one not previously registered in this sweepstakes) who subsequently registers and becomes a prize winning contestant. A referral of someone who is already registered in the $1 Thousand Dollar Birthday GameTM is simply void and of no effect. In the case of a website, a website will be considered a referring website if and only if a person links to the EZsweeps.com TheBirthdayGame.comTM page directly from the affiliate site, and then without leaving the TheBirthdayGame.comTM site, registers in the sweepstakes, activates their Personal Portfolio and is subsequently a prize winner. In the case of an individual, you will be considered a referring person if and only if a contestant responds to and registers directly from an email sent as a result of the "tell a friend' program on the Ezsweeps.com website, activates their Personal Portfolio and then is subsequently a prize winner. In the event of multiple referrals of the same individual, the only person eligible for the referral prize is the person whose name appears on the email to which the contestant responds. In the event a selected contestant does not win the a prize, no consolation prizes will be awarded to the referring website or person.

Grand prize winners will have ten days to claim their prize and must present proper identification to ID Promotions, Inc. after the documented win. ID Promotions, Inc. reserves the right to extend this ten day period to another specified time to accommodate the winner in the event of an unusual circumstance.

A birth certificate, driver's license or other identification which is counterfeit, illegible, tampered with or otherwise altered will not be honored and the contestant will be disqualified and ineligible to win. Another finalist/contestant may be selected in this event.

The odds of being selected as an official finalist/contestant depend on the number of entries received. The odds of matching the predetermined winning birthday are 1:26,645. All federal, state and local laws apply. All taxes are the sole responsibility of the winner. Winner will be required to complete IRS form W-9. The purpose of this form is to gather the necessary information to complete form 1099-MISC. By signing the W-9 the winner will be attesting to its accuracy. Ascendant Media will file form 1099-MISC with the Internal Revenue Service and furnish the sweepstakes winner with a copy of this form. These forms are to be mailed to the sweepstakes winner and to the IRS no later than the end of January and February, respectively in the subsequent calendar year.

Sweepstakes information and winner's names can be obtained by sending a self-addressed, stamped envelope along with the request to Ezsweeps.com, 1770 W State St. #254, Boise, ID 83702. The actual predetermined annual winning birthday will be available after July 1, 2008 (or current contest year).
